HOW TO CONDUCT 1st Training

How to Schedule, Set up, and Conduct 1st Training

1- SCHEDULE 1st TRAINING with New Employee

  • 1st Training is 3 hours long and is best timed around a meal
  • Inform them of the date and time of the next STAFF MEETING.
  • Plan to set up 2nd Training and Overnight Training at 1st Training

2- Send EMAIL (with 5 attachments) to get them started before 1st Training 

SUBJECT: Welcome to our Caregivers Team!

Dear ______,
You are joining an amazing team of fun, caring individuals that provide around the clock care for Don Taylor. Because Don's treats everyday as an adventure, he enjoys having lively, equally adventurous people surrounding him. So we think you will really fit in and enjoy being a part of this team.

First we will have to get all the boring paperwork completed, then you will read through the Orientation materials before starting your first day of Training this ________________________.

You can expect at least 3 Hands-on Training shifts before you (and Don) decide you are ready to work alone with him. Even our licensed Certified Nursing Assistants complete this training before beginning training for the CNA visits that get Don in and out of bed, take care of personal and intimate care, toileting, and showering. 

During your first Training Session we will schedule your next training session- so bring your calendar.

At this point we should already have-
 ~Your Resume
 ~Your Reference Sheet
 ~A copy of your Drivers License
~A copy of your Colorado CNA License (if applicable)

We will also need another form of legal ID such as a copy of your 
 ~Social Security Card, 
 ~Passport ID page,
 ~or Birth Certificate.
So either email this to me or bring it to your 1st Training.

And I need an email from you stating 
 ~your best local mailing address, 
 ~best phone number, 
 ~best email address, 
 ~birthdate and 
 ~emergency contact information.

Below there are FIVE attachments for you-
1. Employment Contract between you and Don
2. IRS Form W4
3. US Citizenship Form I9
4. Direct Deposit form
5. Sexual Harassment form
Please PRINT, read, complete, and SIGN each of these forms.

BRING TO YOUR FIRST TRAINING
A SIGNED COPY of each of the 5 forms above.
Your schedule/personal calendar to set up your next training sessions.
A copy of another form of legal ID (unless you already emailed it)
AND a VOIDED CHECK from your checking account to complete your set up for direct deposit of paychecks.

FINALLY-
Before your 1st Training go to http://dontaylortraining.blogspot.com// and read the first 8 entries to this Blog. Don't worry you will not be quizzed on this material. It is all background info for your reference. You can reread it anytime on this blog site.

There will be other (but fortunately much less!) blog entries to review before your 2nd Training, and your Overnight Training.

3- I9 form
4- Direct Deposit
5- Sexual Harassment Policy


3- Receive and PRINT Vital Statistics Email from New Trainee

Once received, PRINT EMAIL with address, phone, email, birthdate, and emergency contact.
If applicable, PRINT any attachments- social security, passport or birth certificate.

4- PREPARE Employee Folder for this person-

  • Use label maker to print last name, first name and put on manila file.
  • Add resume, copy of drivers license, and copy of Colorado CNA license (if applicable) to file
  • Add printed EMAIL with address, phone, email, birthdate and emergency contact to file
  • Add printed ATTACHMENTS as well.

5- BEFORE 1st Training

GOTO USB Stick/1STAFF/New Employee files. 
PRINT the following 5 Applicable Files.
  1. Training Checklist
  2. Checklist PA (or CNA)
  3. PA 1st Time Alone
  4. CKLIST On Arrival
  5. CKLIST Before Leaving
GET previously prepared Employee Folder for this person.



PREPARE 2nd folder with first name only for WORKLOG

Prepare Worklog from USB Stick/1STAFF/Worklogs EXCEL file "Worklogs" and put in folder

Add Checklist PA (or CNA) handout to WORKLOG FOLDER

Add PA 1st Time Alone handout to WORKLOG FOLDER

Place WORKLOG FOLDER alphabetically in wall file at Caregivers Desk.

6- AT 1st Training


COLLECT signed paperwork-
  • Contract
    • (Sign and date this for Don)
  • W4 form
  • I9 form
  • Direct Deposit form
  • Sexual Harassment Policy
  • and any other paperwork they brought in person.
MAKE COPIES of-
  • Contract (Copy goes to New Trainee, Original to Employee Folder)
  • Sexual Harassment Policy (Copy goes to New Trainee, Original to Employee Folder)
  • W4 form (Copy goes to Accountant, Original to Employee Folder)
  • Direct Deposit form (Copy goes to Accountant, Original to Employee Folder)

EMPLOYEE FOLDER should have-
  • Resume
  • References
  • COPY of Drivers License
  • COPY of Colorado CNA License (if applicable)
  • Printed EMAIL with best address, phone, email, birthdate, and emergency contact
  • COPY of Social Security card, Passport or Birth Certificate
  • ORIGINAL of Signed Contract
  • ORIGINAL of W4 form
  • ORIGINAL of I9 form
  • ORIGINAL of Direct Deposit form
  • ORIGINAL of Sexual Harassment form
The following COPIES go to Don's Accountant- Marty
  • COPY of W4 form
  • COPY of Direct Deposit form
The following COPIES go home with New Employee-
  • Signed Contract
  • Sexual Harassment Policy
WORKLOG FOLDER should have-
  • Worklog for New Employee
  • PA (or CNA) Checklist handout
  • PA 1st Time Alone handout
